Discovering a new normal - empowering post-op laryngectomy patients through a multidisciplinary education program

Abstract: A total laryngectomy is a life-altering operation, literally and figuratively. The patient's successful recovery is dependent not only on the competence of their providers, but also on their confidence in caring and advocating for themselves. A speech-language pathologist's expertise in head/neck anatomy and physiology, swallowing, and communications differences prime us to play a key role in the education and empowerment of laryngectomy patients. The goal of this presentation is to detail a multidisciplinary approach to post-op patient education and training, with a focus on patient independence in preparation for post-acute life. Participants will hear from inpatient speech-language pathologists and their head and neck oncology nurse navigator regarding their experiences in preparing patients for successful discharge post-laryngectomy.

Description: The 30-day unplanned readmission rates for post-op total laryngectomy patients varies, but averages about 20%. Our goal is to support clinicians in developing confidence in post-laryngectomy education and training and to promote successful discharges via patient independence. We will focus on reviewing anatomical, physiological, and lifestyle changes that accompany this surgery, as well as detailing our facility-specific post-op laryngectomy discharge education checklist. The following will be topics of discussion:
- Pre- and post-op anatomy
- Benefits of total laryngectomy
- Developing reliable form of post-op communication
- Introduction of peri-stomal supplies and care (i.e. stoma, HME, Lary tube)
- Understanding patient/family learning styles
- Impact of post-acute oncology plan of care (i.e. chemoradiation, etc)
- Review of discharge skills checklist
In our facility, we rely primarily on our speech-language pathologists, respiratory therapists, nurses, and nurse navigator to ensure the patient and their family/caregivers have met criteria for a safe discharge. This includes peri-stomal and airway management, reliable communication, and return demonstration of laryngectomy-specific education. A rather important topic of education includes ensuring patients are able to self-advocate in emergency situations as "neck-breathers". It is imperative that our patients are able to convey their anatomical differences to other healthcare providers in the event of an emergency.
